Systematic review: Enhancing Breast cancer-related lymphoedema_ Management with Complete Decongestive Therapy and AI-Driven Physical Therapy Innovations

Abstract
Background:
breast cancer-related lymphedema (BCRL) affects 30-50% of patients’ post-axillary lymph node dissection.Rehabilitation,Complete Decongestive Therapy (CDT),and artificial intelligence (Al) are cornerstones of management.This review evaluates current evidence and future directions.
Objectives:
Primary Objectives:Postpone the onset of breast cancer-related lymphedema (BCRL) through: patient training,rehabilitation,and skin care,Compare non-surgical as compression therapy &lymphatic massage and surgical approaches as veno-lymphatic anastomosis&lymph node transplantation in handling post-mastectomy lymphedema.
Secondary Objectives:Define post-mastectomy lymphedema by:causes,manifestations,and guidelines.
Methods: Studies from PubMed Central,Frontiers in Oncology,CochraneReviews, oncology,integration of data.
1. Electrotherapy for cancer:(inclusion&exclusion criteria)
Inclusion: electrotherapy dealing with cancer(asIRE,TTFields,Electrochemotherapy)
Exclusion:unproven studies that are not related to electrification have been excluded.
Outcomes:The efficacy of IRE in destroying cancer cells, immunostimulation, electro-nanotechnology as TENG,improved survival and reduced complications.
2. Al&Lymphedema(inclusion&exclusion criteria)
Inclusion:the studies applying Al in diagnosis,and treatment.
Exclusion:editorials,reviews,abstract-only papers.
Outcomes:AI detect lymphedema with >90% aaccuracy,use ML to identify risk of infection,guide qualifying exercises using AI.
3. AI in Breast Cancer Diagnosis:(inclusion&exclusion criteria)
Inclusion and exclusion criteria are incompletely explained,but include clinical and practical applications.
Outcomes:AI Improves early detection accuracy in radiography and mammogram,support in treatment decisions by analytical data from images and tissues.
4. AI in Medical Imaging:(inclusion&exclusion criteria)
Inclusion:Articles on AI and medical imaging from 2017 onwards
Exclusion:incomplete or indirectly related to medical imaging
Outcomes:AI improves the accuracy of diagnosis,use CNNs and Transformers in analyzing radiation images with high resolution.
Results:
Non-surgical interventionsparticularly Complete Decongestive Therapy (CDT),show significant effectiveness in improving functional capacity
Aquatic therapy,according to 2024 study in Lymphatic Research,offers a safe approach by utilizing hydrostatic pressure to reduce lymphatic swelling.
According to 2025 studies from Germany and Edinburgh,AI achieved 90–100% accuracy in detecting Stage 1 lymphedema.
Early detection using advanced tool and integration of both physical and technological components.
Conclusion:
Complete decongestive therapy,Integrating AI,PT and surgery optimizes outcomes,minimizes risks .
